users being deleted

We have users being deleted from our server. Can not find anything in the logs. Has anyone ran into this before.

This has been reported by a few users lately. We are currently investigating the cause of this. However, we ideally need to reliably replicate this bug to track it and fix it so any information on what seems to be the difference between accounts that are deleted and ones that aren't would be useful. For example, does changing the password or other settings of the user seem to cause them to be lost from the system? Are you using the normal file system or ODBC?
